Mr. Coleman's Story

What do today’s seniors need most from their doctors? Time, understanding, and coordinated care. That’s where CenterWell's value-based care model comes in. In a value-based model, doctors are focused on achieving the best health outcomes for their patients, rather than being incentivized by the volume of patients they can see in a single day.

For 73-year-old Michael Coleman, his experience with CenterWell Senior Primary Care has been a game changer. There aren’t long wait times, rushed doctor’s visits or health care staff that don't follow through. Instead, he’s able to get the support and level of care that he needs and has a deeper connection to his community too.

“Another thing I like about CenterWell [is that] it’s not just for patients. It’s for the neighborhood," says Coleman.

Daniel's Story

Sometimes, all it takes is one healthy action to inspire another. Take Daniel, for example.

When Daniel, a Humana Medicare Advantage member, realized he'd hit 30 years as a smoker, he knew he needed to make a big lifestyle change to be the healthiest he could be. He tried several times to quit by himself, but nothing stuck. Could help from his health insurance actually be the way he'd finally break the habit?*

Find out how Daniel was able to quit for good by reading his story at https://huma.na/47APiaF.

Patrick's Story

A machinist, Patrick has a gift for fixing things. So when he lost the use of his leg, he modified his truck to control the gas pedal with his hands. Then later, when he lost the use of a hand, he added a steering wheel ball so he could keep driving. Patrick was determined to stay independent. But staying optimistic was harder.

When Patrick got his Humana Medicare Advantage plan, his sales agent, Darlen, asked what he needed. And he said, “I need a leg so I can start to walk again.” Now with a little help from Humana, Patrick is once again lifting himself up.

Joaquin's Story

Helping people overcome their challenges—that's just what we do. For Joaquin, a 75-year-old Humana member living with end-stage kidney disease, that meant finding the help he needed to get to his life-saving dialysis appointments.

After a hospital stay left Joaquin too weak to walk on his own, he didn't know how he was going to get up and down the stairs to his apartment or get to his treatment center. That's when Humana connected Joaquin with an in-network ambulance service that helped him get to his dialysis appointments.*

Because Joaquin got the dialysis he needed, he hasn't had to go back to the hospital, and he's been able to regain his strength. Today, he's able to walk again and leaves his apartment whenever he wants.**

The Shingles Vaccine Is Good News for People With MS

At a recent doctor's appointment, I learned about a new shingles vaccine. On October 20, 2017, the Food and Drug Administration (FDA) approved Shingrix for the prevention of shingles (herpes zoster) in adults aged 50 years and older. Shingrix is a non-live, recombinant subunit vaccine given intramuscularly in two doses, spaced two to six months apart. For full effect, it’s important to complete the course and receive both injectionsLess than a week later, the Advisory Committee on Immunization P...

Medication Adherence Star Ratings Measures, Health Care Resource Utilization, and Cost

Following medication schedules as prescribed can have an impact on more than just health—it can affect healthcare use and costs, too.

A recent Humana Healthcare Research study found that inconsistencies in medication adherence were associated with increased likelihood of healthcare resource utilization, such as emergency room visits and inpatient stays, and increased costs for patients taking medications to treat diabetes, hypertension and hyperlipidemia.
